If you are an HMO member, please bring the following during your visit. Please note coverage and approvals may vary depending on your HMO plan. Our team will assist in coordinating with your HMO
provider when needed.

  • Valid HMO ID

    Please present your active HMO card. If you do not have your card with you, you may provide your HMO membership number so our staff can verify your eligibility and coverage.

  • Government Valid ID

    A valid ID is required to confirm your identity and match your HMO membership details.

  • Approved Letter of Authorization (LOA)

    Some HMO providers may require a Letter of Authorization before consultation or diagnostic procedures. Please secure this from your HMO provider if necessary.
